Exercise as part of reflux treatment: CPMPK participates in the TRAIN study

Gastroesophageal reflux disease is a very common condition that can significantly affect quality of life. It is typically manifested by heartburn, pressure behind the sternum, or the return of stomach contents into the esophagus. In addition to lifestyle changes and pharmacological treatment, the importance of targeted physiotherapy is increasingly being demonstrated.

The TRAIN (Treatment of Reflux diseAse usINg targeted physiotherapy) study focuses on this area, examining the effect that diaphragmatic breathing training can have on problems associated with gastroesophageal reflux disease.

How diaphragmatic breathing can help

The diaphragm is not only the main respiratory muscle. It also helps stabilize the torso and plays an important role in the anti-reflux barrier - a system that prevents the return of stomach contents into the esophagus. Together with the lower esophageal sphincter, it helps maintain the correct pressure mechanism between the stomach and esophagus.

Targeted diaphragmatic breathing training can improve its function. Current evidence suggests that patients who learn and practice this technique regularly may experience a reduction in reflux symptoms and, in some cases, a reduction in the need for anti-reflux medication.

How the study is conducted

The study is designed for a period of 15 months and consists of three main phases:

  • control phase (3 months) – usual reflux treatment, lifestyle changes and monitoring of problems,
  • active phase (3 months) – diaphragmatic breathing training under the guidance of a physiotherapist,
  • maintenance phase (9 months) – regular practice of learned techniques and gradual evaluation of the effect.

The study also includes specialized examinations of the esophagus and regular assessment of problems using short questionnaires.

Physiotherapy according to DNS principles

Diaphragmatic breathing training is conducted under the guidance of a physiotherapist and uses the principles of Dynamic Neuromuscular Stabilization (DNS) according to Professor Pavel Kolář. Patients undergo a series of individual lessons during which they learn to properly engage the diaphragm and the trunk stabilization system.

Who is the study intended for?

The study is intended for patients with gastroesophageal reflux disease who have long-term difficulties despite lifestyle changes or pharmacological treatment.

Participation in the study brings the opportunity to receive highly specialized physiotherapy and at the same time monitor its effect through objective medical examinations.
